HOW DO I SIGN UP FOR ONE YOUR PROGRAMS? 2018-01-17T21:00:39+00:00

Most of our programs are accessed by one of three ways: the patient can sign up themselves, the doctor or health professional can refer them, or they may be contacted by us to join. For certain services, like our mental health program, you may need a referral from your Family Physician or other health care provider so check out our programs and services page to find out how you can sign up.

WILL I STILL GET TO SEE MY DOCTOR IF HE OR SHE IS PART OF A FAMILY HEALTH TEAM? 2018-01-17T21:00:21+00:00

Yes. The Family Health Team is a group approach to health care, your doctor will now be able to focus on your complex medical problems but you’ll have access to different health care professionals according to your needs. Because all of our health professionals work together and are co-located, even though you may be seeing many of the Family Health Team’s providers, they will all be aware of the different components of care you are getting.

Yes, all of the doctors in Wawa are part of the Wawa Family Health Team. You can access many of our services by speaking with your family doctor about the programs we offer.

WHAT WILL THE FAMILY HEALTH TEAM MEAN TO ME? 2018-01-17T20:59:42+00:00

Family Health Teams are patient centred and you will have access to a number of primary health care services from a variety of team members, including your family doctor.

For example:

  • Your family doctor will continue to see you during regular office hours.
  • You will have access to other health care professionals within the Family Health Team, such as nurse practitioners, nurses, dieticians and pharmacists.
  • If you see another health provider at the Family Health Team for treatment, your own doctor will receive documentation about the appointment so that he or she will always be up-to date about your medical condition.

A Family Health Team is a primary health care model that brings together different health care providers to co-ordinate the highest possible quality of care. Our team is designed to give family doctors support from other complementary professionals like nurses, nurse practitioners, social workers, dietitians and other health care professionals who work together.
