The Wawa Family Health Team is collaborating with Algoma Ontario Health Team, Northern Ontario School of Medicine and Dr. Gilles Arcand Centre for Health Equity to complete a research study. The research includes asking clients to complete a survey on their experiences with the Wawa Family Health Team.
The objective of the survey is to understand the experiences people have when they are accessing care and the affect it can have on their overall health and wellbeing. This study will assess some experiences you had when attending Wawa Family Health Team and combine those answers together with other answers to see if some groups of people have different experiences than others.
In completing the Survey you have a chance to with a $50.00 grocery store gift card. This survey can be completely anonymous.
