Geriatric care provides personalized treatment plans, support with daily activities, and access to resources that enhance physical, mental, and emotional health to our catchment area of Wawa, Dubreuilville, White River, Michipicoten First Nations and Missanabie First Nations.

Advance Care Planning (ACP) is the process of reflecting on and documenting your wishes for future medical treatment in case you become unable to make decisions for yourself. By considering what kind of care aligns with your values and communicating these wishes to someone you trust, you ensure that your treatment will reflect your personal goals and beliefs. ACP offers peace of mind, allowing your loved ones to confidently make decisions on your behalf during difficult times, ensuring your voice is heard even when you cannot speak for yourself.

PROGRAM ON HOLD

An inter-disciplinary approach to providing early diagnosis and treatment for problems related to memory loss. Early diagnosis and treatment can help to maintain independent living and quality of life for as long as possible. You and your family members will meet with a team including a Nurse, Occupational Therapist, Dietitian, Pharmacist, Physician and an Alzheimer’s First Link Coordinator for a comprehensive assessment including driving safety. This information can provide you and your family practitioner with a better understanding of your condition along with steps you and your family can take in your care.
