If you've visited the Wawa Family Health Team before, you may know the consistent question our Administrative Staff ask every time, "Do you have your Health Card?".

Your health card is an important piece of identification our clinic uses to verify booked appointments. But your health card is more than just identification, our administration ensures all Health Cards are up to date in our electronic medical record system by updating the latest version code and expiration date.

Did you know Health Cards ensure you have OHIP covered care, instead of paying out of pocket. They are also important for your Physician or Nurse Practitioner to refer you to specialists, and access to certain testing for your health.

Not Up-to-Date

If your health card is not up to date, you can expect a request to renew ASAP. You can find out how to renew on our FAQ page "How do I renew my Health Card". You can expect delays or rejection from Specialists, the inability to order some tests, or you may receive an unexpected bill in the mail.

Updated Health Cards

A paper copy or photograph of your health card can be accepted and is valid at our Family Health Team.

If you update your health card, you can phone us with your updated health card information or bring it in at your earliest convince.

The Wawa Family Health Team would like to acknowledge the land in which we hold our practice of medicine on and the land of the people we serve. We are grateful to reside on the lands and home of the Anishinaabe and Cree and respectfully acknowledge that this land is Indigenous land that has been inhabited by Indigenous people from the beginning. We are grateful for the opportunity to connect here, and we thank all the generations of people, who have stewarded and cared for the land for thousands of years. We recognize and deeply appreciate the historic contributions of the First Nations, Inuit, and Metis peoples both in shaping and strengthening this province and country. The Wawa Family Health Team promotes a healthier community through creating a safe space that welcomes diversity, inclusion, and acceptance for everyone. We are committed to the challenge of Truth and Reconciliation, and we are learning that acknowledgement of the past and current traumas will aid us in providing equitable and culturally appropriate care. We promise to express humility in our daily practices to provide the utmost patient-centered care we can.
