Booking An Appointment
HOW
To book an appointment at the Wawa Family Health Team, you can call us at 1-705-856-1313 or your can book online by creating a profile and password with Medeo, our online platform.
WHO
If you have a family doctor, please book an appointment with them. If you do not have a family doctor, please book with a locum doctor or other team provider if a referral is not required.
WHY
Appointments at the clinic are not for emergencies. If you have concerns and would like to assess situation please call 811 for Telehealth advice. If it is an emergency please call 911 for further instructions.
Refilling A Prescription
Ensuring your medication is uninterrupted is an important factor to your health. Please ensure you book an appointment if a refill if needed a month in advance of running out. Before booking an appointment to refill your prescription, please follow these steps:

Check your prescription bottle label to see if there are any that can be called into the pharmacy directly.

Call your pharmacy directly to ensure there are no available refills if not indicated on the prescription label.

Book an appointment with your family doctor, locum, or nurse practitoner noting that it is for a refill.
New Patients
For your visit you should bring:

Your Ontario Health Card

Name, Address, and Telephone # of someone we can contact in the event of an emergency

A list of medicines you are taking now

A list of your relevant medical history (chronic conditions, allergies, immunizations, previous illnesses and/or surgeries)

Any letters, reports, tests, or x-rays that your referring doctor may have asked you to bring
The Wawa Family Health Team has a wealth of health care professionals here to care for you. While a family doctor may not be possible, our collaborative team works together to deliver the best care for our clients. We look forward to meeting you!
Renewing An Ontario Health Card
To provide you with health care, you have to have a valid Ontario Health Card. Ensure you have the card with you for your first visit, and it is renewed upon its expiry.

Emergency
Services
The Lady Dunn Health Centre has an ER for emergency situations. Dial 911 for ambulance dispatch, do not drive yourself or others if it is unsafe to do so.
Take note of where you are, stay calm and provide information to the operator. They will provide advice and further instruction once the situation is assessed.
